Christ Community Health- Behavioral Health Services

Behavioral Health Consultants are trained as psychologists, licensed clinical social workers or licensed professional counselors. They work with primary care physicians and spiritual health advisors to enhance care for those patients suffering from emotional issues and chronic medical conditions. Patients are offered an opportunity to meet with a Behavioral Health Consultant for a brief assessment and targeted intervention as part of their general primary care visits when clinically necessary.

Christ Community Health- Family Solutions

Family Solutions is staffed with Licensed Therapists and Psychologists, that offer families a judgment-free zone to navigate the many challenges they may be face with their children and within the family. Parents and children can have access to individual counseling, family counseling, and support groups to assist in managing everything from developmental challenges, and trauma to Mental health conditions. In addition, Family Solutions offers a variety of specialty alternative therapies such as Mindfulness Meditations, Yoga, Physical stress-relieving activities and informational workshops designed to equip families with the solutions needed to create a healthy and supportive environment for the children.

Christ Community Health- Social Work

Patients are offered the opportunity to meet with a Social Worker during the patient’s visit or consult with a Social Worker by phone. Social Workers assess for the needs such as Food Insecurity, Clothing Needs, Housing Issues, Transportation needs, Medication Assistance Information, Employment Resources and much more. In addition, Social Workers assist our patients by connecting them with resources offered within our community. Social Work Services are offered to patients who see providers at all of our Christ Community clinics.
